What’s included?

  • 90 Minute Onboarding Consultation to review your history, lab tests and current diet.  The purpose of this session is to determine what nutrition interventions will be most effective at addressing the root cause of your CKD to slow down, or even halt its progression. We will establish what these nutrition priorities are for you and discuss a general plan. Opportunity to ask key pressing questions and get clarity on concerns.

 

  • Nutrition Action Plan This is your personalized nutrition and lifestyle plan.  It will include specific recommendations on what to eat, what to avoid, what to watch out for, and many other important details to pay attention to. Following this personalized nutrition plan closely will be essential  to achieving the results you seek.
  • Meal Plan containing at least 8 recipes (breakfast, lunch, dinner, snacks). These recipes will be carefully selected to adhere to both your nutritional needs and your food preferences discussed during the initial consultation.

 

  • Two 30 minute follow-up sessions.   These sessions will be a time to track and assess your progress, review important concepts, and tweak interventions to keep the needle moving towards meeting your goals. Follow up sessions must be scheduled within three months of initial session.
  • 30 minute post-labs follow-up session.  Labs should be checked 60-90 days post diet change.  

 

  • Email support between sessions to help answer general questions. For more intensive support, an additional session will be recommended.
